Changing CARE is the action phase of our caregiver-focused strategic plan. It involved four partnerships from across Ontario.

Young carers

We are convening key people and organizations to help support young carers in the important role they play in health care, in society and at home.

Spotlight On Ontario's Caregivers

This 2nd annual report on the state of caregiving in Ontario is the first done in collaboration with The Ontario Caregiver Organization and the first to start highlighting trends in the caregiver experience in Ontario.

Integrated care

Together with our partners, we created the first virtual collaborative on Integrated Care for North America, allowing us to share with, and learn from, each other.

Primary care

The Primary Care Virtual Community brings together leaders in primary care who are interested in supporting meaningful change in Ontario’s healthcare system.

Past Initiatives

The Caring Experience

The Caring Experience was instrumental in allowing us to better understand the experiences and interactions, as well as the common ground, between family caregivers and those working in Ontario’s health and community care sectors.

PANORAMA

PANORAMA was a standing panel of 31 Ontario residents exchanging views, experiences and advice that helped The Change Foundation in its work to improve the patient/caregiver experience.

Partners Advancing Transitions in Healthcare (PATH)

PATH engaged and supported a community coalition of cross-sector providers and seniors/caregivers to redesign how care is delivered, addressing problems that were identified together.

20 Faces of Change

This 2014 campaign honoured 20 teams, individuals and organizations who contributed to improvements in patient- and family-centred care in Ontario.

Long-Term Care Resident Councils and Family Councils Project

This project delivered a series of best practices and recommendations for how LTC facilities and resident and family councils can work together effectively.

Experiencing Aging: A Health Care Reality Check

Through the use of the Frail Aging Suit and simulations informed by lived experience from patients and caregivers, we crafted a videos series that serves as a reality check of the challenges seniors face in accessing health care.
