When a family member is sick, family caregivers face many challenges and obstacles while trying to care for their loved ones, navigating a complex system of healthcare providers and organizations that lack connectivity for caregiver transitions as well as an understanding and appreciation for the vital role caregivers bring to patient care teams.
Connecting the Dots…Smoothing Transitions for Family Caregivers is focused on addressing the needs of caregivers through defining and recognizing their role by working with caregivers and care providers to co-design systems of care provision and communication. The ultimate goal is to improve the quality of care and experience for caregivers, patients and health care providers.
The partnership will include educational supports for caregivers and health care providers, as well as the development of caregiver assessment tools so that their needs can be identified and met early to avoid burnout. Technology will be developed and used to enable seamless communication and enhanced service coordination between caregivers and various care providers, which will assist in closing the gaps between transitions in different care environments.
Over the course of the partnership, health care providers, family caregivers, and administrators will work together to address Changing CARE’s four interconnected themes: communication, assessment, recognition, and education.
These partner organizations have a long, successful history of collaboration to enhance the quality and coordination of care throughout Huron and Perth Counties. Family caregivers from each of the partner organizations will work alongside healthcare providers in every phase of Connecting the Dots, which will change the culture of care delivery through new dialogue, partnerships and improved interactions with caregivers in the community.
