Embrace, building on a partnership between the Cornwall Community Hospital and Cornwall & District Family Support Group has been on a journey to improve interactions between family caregivers and providers in the addictions and mental health system. Hear from Joanne Sidorchuk, Program Manager, Jeanette Despatie, President and Chief Executive Officer and Micaela Harley, Youth Advisor & Project Co-lead (Young Carers) share the impact of Changing CARE.
Improving CARE Together brought together all of St. Joseph’s Health Care London five sites to enhance and accelerate the work that was underway to engage with and build robust partnerships with patients and caregivers. Complementary to the corporate vision and strategic direction, hear from Roy Butler, Vice President, Patient Care & Risk Management, Jacobi Elliott, Project Lead and Dr. Gillian Kernaghan, President and CEO share how Changing CARE has enabled the opportunity to embed care partnerships from principle into practice.
Connecting the Dots, true community partnership, brought together Huron Perth Healthcare Alliance, ONE CARE Home and Community Support Services, North Perth Family Health Team (FHT), STAR FHT, the South West Local Health Integration Network and the Alzheimer Society of Perth County to improve the quality of care and experience for family caregivers, patients and health care providers in Huron and Perth counties in Southwestern Ontario. Hear from Andrew Williams, President & CEO, Charlene O’Reilly, Caregiver and Co-Design Team Member, Barb Fewster, Caregiver and Co-Design Team Member, Mary Atkinson, Executive Director, North Perth FHT, share the value of Changing CARE on fostering awareness of the caregiving role to enable valuing caregivers as members on the circle of care.
Cultivating Change brought together Sinai Health (SH) and WoodGreen Community Services (WG) to reimage the way hospital care and community services are designed to honour caregivers as partners in care. Hear from Ashnoor Rahim, Vice President, Community Care (WG), Akiko Okamoto, Process Improvement Consultant (SH), Dr. Gary Newton, President and CEO (SH) share how Changing CARE facilitated a fundamental culture shift where caregivers are formally identified, valued, supported and actively partnered with.
